Drug approved to treat cystic fibrosis' root cause

(AP) ? The first drug that treats the root cause of cystic fibrosis won approval Tuesday, offering a life-changing treatment for a handful of patients with the deadly illness and broader hope for thousands more patients with the inherited disease.

About 30,000 Americans live with cystic fibrosis, a disease that causes sticky mucus buildup in the lungs and other organs, leading to infections, digestive problems and death in young adulthood. The typical life expectancy is about 37 years, according to the Cystic Fibrosis Foundation.

The Food and Drug Administration approved Vertex Pharmaceuticals Inc.'s Kalydeco for patients with a rare form of the disease that affects just 1,200 people in the U.S., about 4 percent of affected population nationwide. These patients have a protein defect that prevents their cells from properly absorbing and excreting salt and water. Studies of the drug showed it significantly improved lung function and reduced other symptoms of cystic fibrosis.

"Even though this drug isn't for the majority of people, it proves that you can look at the mistake in the genes and design a drug in a rational way that will fix the problem," said Dr. Drucy Borowitz of the State University of New York at Buffalo, where she directs the cystic fibrosis program.

The twice-a-day pill is among the first drugs designed to a correct a specific genetic defect. Its development characterizes both the promise and challenges of that approach. Scientists first identified the gene that causes cystic fibrosis in 1989, but it took more than two decades and more than $75 million in outside funding to develop a drug to treat the disease.

Borowitz enrolled several of her patients in the key study for Kalydeco, which showed that patients taking the drug increased their lung strength more than 10 percent when compared with patients taking a placebo. Patients also had fewer infections and gained nearly seven pounds on average, a significant amount for patients who typically have trouble retaining weight. All patients in the study continued taking older medications that help loosen mucus.

"Two weeks after using the drug my lung tests were above average for a healthy 15-year-old who didn't have cystic fibrosis," said Nick Mangano, 17, a Borowitz patient who has been taking the drug for two years. Before starting on Kalydeco, Mangano said he was hospitalized for lung infections five times in four years. Now he says he usually recovers from a cold within a week or two.

"I don't really need medicine for it anymore, it's totally different," said Mangano, who is considering leaving Buffalo for college next year ? a step he hadn't previously considered because of his dependence on his family and physicians.

Only a few decades ago, children with cystic fibrosis seldom survived elementary school. Today, thanks to earlier diagnosis and new focus on diet and physical therapy, 47 percent live to be 18 or older.

The FDA approved the drug for patients 6 years old and up, though Vertex is also planning to study the drug in patients as young as 2 years old. Researchers hope that by using the drug earlier they will be able to prevent permanent lung damage, which is the primary cause of death for cystic fibrosis patients.

Mangano and others with the so-called G551D mutation have a defective protein that fails to balance the flow of chloride and water across the cell wall, leading to the buildup of internal mucus. The vast majority of cystic fibrosis patients have a different genetic defect, in which the protein does not reach the cell wall. Vertex is developing another drug to try and address that problem. Study data for that drug is expected later this year.

Kalydeco is part of a growing number of new medicines that target rare genetic variations found in subgroups of patients. Last year Pfizer launched a new lung cancer drug called Xalkori, which targets cancer linked to a genetic mutation found in less than 7 percent of patients.

Vertex executives said Kalydeco would cost $294,000 for a year's supply, placing it among the most expensive prescription drugs sold in the U.S. Specialty drugmakers are known to charge $300,000 or more for drugs that treat very small groups of patients.

"The drug is priced for the value it will deliver to this very small group of patients," Vertex Executive Vice President Nancy Wysenski told analysts.

Wysenski said Vertex would provide the medicine for free to people with no insurance and household income of $150,000 or less. The company will also cover 30 percent of copay costs for select patients who have insurance.

The FDA approved the drug in approximately three months, half the time usually needed for high-priority drugs.

After scientists identified the genetic sequence that causes cystic fibrosis in 1989, many experts hoped the disease could be cured by replacing the gene with a normal one. However, attempts at so-called gene therapy proved unsuccessful, and researchers began looking for ways to correct the genetic defect.

"I think it took the field about a decade to realize we had to look for other options," said Paul Negulescu, vice president of research at Vertex Pharmaceuticals.

In 1998, the Cystic Fibrosis Foundation approached Aurora BioSciences, now part of Vertex, to help screen potential drug candidates for a cystic fibrosis drug. In 2000, the foundation awarded the company more than $45 million to study and commercialize an experimental drug for the disease, the largest grant of its kind by a nonprofit disease group. To date, Vertex has received over $75 million in research and development funding from the Cystic Fibrosis Foundation. Vertex said it also spent hundreds of millions of dollars of its own money, though it did not specify the exact amount.

Cambridge, Mass.-based Vertex has only one other drug on the market, the hepatitis C drug Incivek, which launched last May.

Company shares rose $2.21, or 6.4 percent, to $36.95 in trading Tuesday.

The most common side effects with Kalydeco include headache, stomach ache, rash diarrhea and dizziness.

Acer K330


The Acer K330 ($600 street) is one of the first examples of what's rapidly turning into a new category of projectors. Much like most palmtops, it's built around red, green, and blue LEDs and a DLP chip. However, it's bigger and brighter, with a 500 lumen rating, and it offers a claimed WXGA (1,280 by 800) native resolution. It is, in short, a capable business projector. Acer also touts it as a home entertainment projector that you can set up quickly to watch movies or play games and then store away when you're not using it.

If the overall description sounds familiar, it may be because the K330 is so similar to the Optoma ML500 ($650 street, 3.5 stars), another sub-3 pound, 500 lumen projector. When I reviewed the ML500, I pointed out that both the price and brightness were modest by traditional sub-3 pound micro projector standards, and suggested that you could think of it as a budget priced micro projector. With the K330, and other similar projectors on the way, the budget micro projector is looking more and more like a significant category.

Basics
The K330 weighs 2.9 pounds?a bit more than the ML500 but light enough to carry around without a second thought?and it measures 1.8 by 8.6 by 6.6 inches (HWD). It comes with a soft carrying case that's large enough to hold the projector as well as its cables and credit card size remote.

Setup is standard, with the back panel offering a suitable array of connectors, including a VGA port for a computer or component video, an HDMI port for a computer or video source, and a composite video port. In addition, there are two miniplug jacks for AV input and audio out, both an SDcard slot and a USB Type A port for reading files from a memory card or USB memory key, and a mini USB port for connecting to a computer to transfer files to the 2GB internal memory,

The memory options help add to the projector's portability by letting you leave your computer or video source at home. According to Acer, the K330 can read more than 20 file formats, including PowerPoint, Word, and Excel files (up to Office 2010); PDF files; video files (MPEG-1, MPEG-2, and 5 other formats); image files (BMP and JPG); and audio files (MP1, WAV, and 7 more).

Brightness and Data Image Quality
The 500 lumen rating may sound anemic compared to lamp-based portable projectors, like the Editors' Choice NEC Display Solutions NP-M300WS ($1,099 direct, 4 stars), with its 3000 lumen rating. However, perception of brightness is logarithmic, so a 500 lumen image looks much more than one sixth as bright as a 3000 lumen image. As a practical matter, the K330 was bright enough to let me run my tests using the 2-meter (78-inch) wide image size I normally use with standard projectors, rather than the smaller images I wind up with when testing palmtops.

The projector also handled our suite of DisplayMate tests reasonably well. As with the ML500, however, I saw scaling artifacts?unwanted extra patterns added to patterned fills, like an area filled with dots?at the claimed native resolution. This simply shouldn't happen.

Scaling artifacts in an LCD or DLP display show up when the display has to add or drop pixels in an image to make it match the number of pixels in the display. The native resolution is supposed to tell you how many pixels are in the display, which also tells you what image resolution you need to use to avoid scaling. As with the ML500, the artifacts suggest that the K330's native resolution isn't 1,280 by 800. When I asked Acer about the artifacts, a company representative said he would check into them, but as of this writing, he has not offered any explanation for them.

Fortunately, the artifacts show only on images with fills of closely spaced dots or lines over a large area, so they won't be an issue for most people for very many images. However, the scaling is also likely responsible for the slight soft focus that I also noticed with text and fine details. On the plus side, the projector did well on most other tests, with suitably neutral grays indicating good color balance, and vibrant, well saturated colors.

One major surprise is that I saw little to no rainbow effect with data images. Rainbow artifacts are a potential issue for any single-chip DLP projector, because of the way the projectors create color. I'm fairly sensitive to the effect, but with the K330, I didn't see it in data images at all. I saw it in video images, but only occasionally and even then fleetingly enough that I might not have recognized them as rainbow artifacts if I weren't so familiar with the rainbow effect. Unless you're even more sensitive to seeing the rainbows than I am, they simply won't be an issue with the K330.

Video Image quality and Other Issues
Image quality for video is best described as usable. It's not something you'd want for a full-scale home theater, but it's appropriate for the kind of casual home use that Acer suggests the projector can be used for. Note, however, that although the K330 claims support for 3D, it's limited to PC-based 3D only.

The one noteworthy issue I saw besides the occasional rainbow was moderate loss of shadow detail (details based on shading in dark areas). The projector handled skin tones reasonably well, and I didn't see any motion artifacts, posterization (colors changing suddenly where they should change gradually), or other obvious problems.

It's worth mention also that the 2-watt mono speaker is loud enough to fill a small conference room, which is far better than most small projectors can manage. And don't overlook the savings you get on total cost of ownership with an LED light source. The 20,000 hour lifetime means the LEDs will last the life of the projector, so you won't have to shell out any money for replacement bulbs.

The Acer K330's mix of small size, low weight, brightness, and image quality makes it a more than reasonable pick if you need a highly portable projector. It should also be of particular interest to anyone who tends to avoid DLP projectors because of rainbow artifacts. The scaling artifacts keep it from being an Editors' Choice, but even with that problem it's attractive enough that if you're looking for a small but reasonably bright micro projector, the Acer K330 belongs on your short list.

(AP) ? Clouds of smoke from Lunar New Year fireworks sent air pollution in Beijing soaring under a new more sensitive measurement system, reports said Sunday.

Readings of fine particulate matter called PM2.5 reached 1.593 milligrams per cubic meter on the Jan. 22 eve of the holiday, about 100 times worse than the amount considered good for 24-hour exposure, the city's environmental bureau said.

The reading drew wide publicity in the local media on Sunday. The popular Beijing Youth Daily praised the city government for taking a more critical look at air pollution, while urging residents to consider the environmental effects of setting off fireworks.

By noon on Sunday, as people headed back to work after the weeklong holiday, the level stood at a relatively good 0.039.

Concern has grown over air pollution from automobiles and other sources, prompting the city this month to begin announcing measurements of PM2.5 ? particles less than 2.5 micrometers in size. That's about 1/30th the width of an average human hair.

Because of their small size, the particles can lodge deeply in the lungs and are believed to pose the greatest risk to health.

Beijing is frequently cloaked in yellow haze, with buildings a couple of blocks away barely visible.

On particularly bad days, schools cancel outdoor activities due to the risk of respiratory problems and hospitals report increased numbers of people ? particularly the elderly ? seeking help because they're having trouble breathing. Long-term exposure can cause a range of illnesses, especially to the cardiovascular system.

The city previously only released readings of the coarser PM10 standard that often indicated pollution was "light," leading to accusations it was hiding the true extent of the problem.

The U.S. Embassy began independently monitoring PM2.5 from a device on its rooftop and releasing the results via Twitter. Some residents even tested the air in their neighborhoods and posted the results online.

Beijing is now releasing hourly readings of PM2.5 that are taken from a monitoring site about 4 miles (7 kilometers) west of central Tiananmen Square.

Madonna says charity plans 10 schools in Malawi (AP)

NEW YORK ? Nearly six years after it was created, Madonna's Raising Malawi charity is set to break ground on the construction of schools in the impoverished country, but they will be run by the local community, not the superstar's organization.

According to organizers, work on the first school will start on March 30 in the Kasungu area, about 80 miles from the capital of Lilongwe, and all of the schools should be built by June 2013. Raising Malawi is providing $300,000 to the non-governmental organization buildOn to develop the schools. They'll serve about 1,000 boys and girls in the southern African nation.

"This remains a very big priority in my life and I am excited that with the help of buildOn we can maintain our ongoing commitment to move forward efficiently," Madonna said in a statement provided to The Associated Press.

Raising Malawi had originally intended to build all-girls schools that the organization would run. But it faced several obstacles in its goal, including complaints from some local farmers that they had been moved off land that Raising Malawi intended to use for its mission. Raising Malawi also had difficulty getting title to the land and there were concerns about the high costs of construction.

The new plan calls for "simple structures" that will be more practical and better serve Raising Malawi's original mission, said Trevor Neilson, who is helping to direct the project as partner of the Global Philanthropy Group. The approach will allow the program to serve twice as many children as before, Madonna said.

"I have learned a great deal over the last few years and feel so much more confident that we can reach out goals to educate children in Malawi, especially young girls, in a much more efficient and practical way," she said. Madonna has adopted two children from Malawi.

BuildOn has already built more than 50 schools in Malawi and 427 schools worldwide.

"For schools to be successful, they need to have community ownership and leadership," Neilson said in an interview Friday. "Raising Malawi shouldn't be running schools in Malawi. Local communities in Malawi should be running those schools, so that's a big part of the shift."

BuildOn has been working in Malawi for almost 20 years, said spokeswoman Carrie Pena. The organization works closely with the community, and locals even volunteer the labor to build the schools, according to Pena.

"It's absolutely a community-owned school," she said.

Neilson praised Madonna for sticking with her plan to build schools for Malawi's children despite several setbacks for the star, who is the director of the new movie "W.E.," out next week, and is this year's Super Bowl performer. Madonna brought in Global Philanthropy to work with Raising Malawi more than a year ago and removed the involvement of the Kabbalah Centre. She has practiced Kabbalah, a form of Jewish mysticism.

"When the previous management team had those problems, I think a lot of people thought Madonna would give up," Neilson said "It would have been understandable, but instead she's going to reaching twice as many kids."

Southwest employee killed in airport accident

A Southwest Airlines employee at Washington Dulles International Airport died on Friday after being injured in a collision with a luggage cart and a vehicle used to transport passengers.

The incident, according to airline spokesperson Brandy King, occurred on Thursday at approximately 7 a.m. ET.

The employee, who has not been named, was rushed to an area hospital and received medical attention. The airline learned early Friday that the employee died.

"We continue to work with the local authorities and the airport to investigate the accident," King said in a statement to msnbc.com. "The Southwest Airlines Family is greatly saddened by the passing of our Southwest Family Member, and we extend our heartfelt sympathies and support to his entire family at this time."

The airline said it is not planning on releasing additional information at this time.

Robotic rat with a monkey's smarts to the rescue?

A Roomba robot outfitted with whiskers and reprogrammed with monkey smarts determines what type of flooring is beneath it.

The next time you find yourself trapped under a pile of rubble, your savior might be a Roomba ??souped-up with whiskers and a monkey brain.

Such a robot was recently shown to outperform other whiskered robots in characterizing its environment, using technology that could wend its way into next-generation search and rescue robots, the University of Sheffiled reports.

Researchers have long known that rats sense their environment with whiskers. But models of how their brains interpret these signals vary.?

One approach, for example, has assumed that rats looked at whisker movement patterns and vibrations over a set period of time and then used that information to make a decision.

But various robots created with this model, Science Now explains, correctly guessed the floor beneath them only 50 to 80 percent of the time, after 0.4 seconds of exposure.

Nathan Lepora at the University of Sheffield in England wondered if outfitting these robots with a model of how the monkey brain makes decisions would be an improvement.

Previous research shows that individual neurons in monkey brains ramp up their firing rates when making decisions about the direction of motion for a group of random dots flashing on a screen.

A decision is made when the firing of these neurons cross a certain threshold. If the neurons responding to the up motion cross the threshold first, for example, the monkey would say the dots are moving up.

Lepora and his team fitted a brain model based on this monkey study into an existing Roomba with rat whiskers and found that it nearly flawlessly?correctly identified the type of flooring?beneath it.

The findings are reported Jan. 25 in the Journal of the Royal Society Interface.

In addition to improved rescue robots, the result suggests that rat brains may function similar to those of monkeys ? in fact, they "suggest the possibility of a common account of decision-making across mammalian species," the team?conclude.

Carl Levin: Mitt Romney criticism of Obama defense budget not factual (+video) (The Christian Science Monitor)

Washington ? Senate Armed Services Committee Chairman Carl Levin says Mitt Romney?s tough criticism of the Obama defense budget is ?just a political statement which is not borne out by the facts.?

In the two most recent Republican presidential debates, the former Massachusetts governor has blasted President Obama?s plans to trim defense spending to comply with spending targets set by the Budget Control Act passed by both parties last August. 

At a Pentagon press conference Thursday afternoon, Defense Secretary Leon Panetta will spell out the Obama administration?s strategy for defending the nation while spending $487 billion less on defense over the next ten years.

Mr. Levin, a Michigan Democrat, was briefed on the plans at a dinner with Secretary Panetta on Wednesday evening. The plan would involve spending about $513 billion, not including the costs of the war in Afghanistan. 

At the most recent presidential debate on Jan. 23, Mr. Romney said Obama's plan to build nine ships a year was inadequate. "We ought to raise that to 15 ships a year," he said.  

At the Jan.16 debate, Romney said, ?The most extraordinary thing that's happened with this military authorization is the president is planning on cutting $1 trillion out of military spending.? He added, ?Our Navy is smaller than it's been since 1917. Our Air Force is smaller and older than any time since 1947. We are cutting our number of troops. We are not giving the veterans the care they deserve. We simply cannot continue to cut our Department of Defense budget if we are going to remain the hope of the Earth.?

After dismissing that criticism as political, Levin told a Monitor-hosted breakfast for reporters Thursday, ?Our military in incredibly strong ... no other military comes close. We have shown our capabilities, our adeptness, our ability to move quickly yesterday in Somalia, before that with Bin Laden.?

Levin said he has spoken with the nation?s top military officers who ?were deeply involved in this budget request and they very much support this budget request not because they have to, but because they were involved in preparing it. They believe it is a sound budget, a strong budget.?

Military leaders are worried about one thing, Levin said. ???The fear that they have is that the sequestration will be triggered. Then you have a totally different ball game.??

Under the 2011 Budget Control Act, there will be additional automatic budget cuts of at least $1.2 trillion over the next ten years, unless Congress acts. The Pentagon would take a major share of those cuts ? some $600 billion starting in January, 2013.

At the breakfast, Levin said he thinks Republicans will vote to raise additional tax revenue to avoid those cuts at the Pentagon. ???The dam has got to be broken on revenues. What will break it I believe is the threat of sequestration."

HBT: It's all about family for Cecil and Prince

Cecil Fielder was making the media rounds yesterday following his son?s signing with the Detroit Tigers. It was understandable given his own history in Detroit.

Thing is, Cecil and Prince have a very complicated history and, in recent years, have been reported to have no relationship at all. The whole story is known only to them, but the?contours?of it seem to be that (a) Cecil was not an attentive father to Prince when he was growing up; (b) his relationship with Prince?s mother was not good; and (c) Cecil is alleged to have squandered Prince?s signing bonus on personal debts. ?Kind of ugly all around.

Cecil was quoted several years ago as saying that Prince had shut him out of his life. Prince, when asked, will not respond to any questions about his father. ?Yesterday, however, Cecil had this to say about the relationship:

?We?re having a few chats. We?re doing a lot better than we were. Time heals all wounds, man. Everybody has to come back together at some point.?

It?s a difficult subject. On the one hand it?s the kind of thing that shouldn?t be any of our business. But it?s been out there because, in the past, Cecil put it out there in interviews and, of course, because both of the Fielders are well-known public figures. And of course now that Prince has gone to the team with which his father found his biggest fame, it?s going to come up a lot more simply because it?s part of a new and wholly understandable dynastic narrative.

Outside of that (and outside of baseball, actually) the subject fascinates me because of what it says about the value and purpose of family.

I have a great relationship with my parents. It?s never been in question because I had a great childhood and they?re good people and all of that. ?But at the same time, I don?t necessarily subscribe to the idea that you have to make a special effort to have such a relationship with family members no matter the situation. If your parents (or siblings or whatever) are jerks or bad people or have otherwise hurt you, I don?t see the need to try any harder to repair that relationship than you would to repair a friendship or another kind of?acquaintance. Or to simply not try at all if that?s your choice.

Yet I feel like I?m in the minority here. I think most people default to the ?but they?re family,? idea, and believe it to be incumbent upon a person to always ? eventually anyway ? try to repair such relationships. And think that a difficult or flawed relationship with a family member is better than no relationship at all.

I can?t see that. Sure, if a family member with whom you?ve had a falling out wants to try and make amends you give that person the same chance that you?d give someone else, but I don?t think you give them considerably more chances or, even further, continue to try to reach out to them out of obligation even if they continue to be a jerk out of some notion that shared blood makes the relationship so much more necessary.

I?m not saying I?m right. I may be very wrong. And like I said before, I have a wonderful relationship with my family so perhaps I?m just taking it all for granted and I?m totally missing the point. I?m curious to hear others? thoughts.

Anyway, the point of all of this is that I don?t see a father-son reconciliation as some necessary component of Prince Fielder going to Detroit. And, even though it would be nicer if the two of them had a good relationship than a poor one, I hope that Prince doesn?t get pestered too much about it by virtue of the public?s need to seek closure or resolution of a relationship that, by all rights, shouldn?t concern us.

Running an automobile repair shop is not an easy thing to do, there are all kinds of laws that have to do with environmental issues, OSHA standards, and what government bureaucrats consider unsightliness that must be complied with.

Starting such a business right now is probably not a good idea because it could cost as much as a half a million dollars to buy all the lifts, secure a location, hire all the people, and do all the initial marketing. That after one has filed all the permits, gone through all the bureaucracy, and jumped through all the hoops.

This is not to say that owning it on a motor repair shop right now isn?t a good thing, it probably is considering the General Motors and Chrysler has cut so much of their auto dealerships, that it has taken out a lot of capacity out of the industry, and there is so much fewer places to get your car repaired.

Supply and demand dictates that this would be a good business to be in right now, even though the economy is down and people are trying to spend less.

Still when people don?t buy new cars, they have to fix up their old cars.

Is there a solution to this problem?

What if you are one of those laid-off mechanics and you want to get into your own automobile maintenance business, but you don?t have the half a million it takes to start your own business and open a repair shop?

One idea is to set up a mobile automobile maintenance business and go to your customers. You know, there are many mobile automobile franchises available, and many of them have financing, and they don?t cost a lot of money to get into. This might just solve all the problems for you. Please consider all this.

Finding the Hawaii of 'The Descendants'

Who hasn?t dreamt of Hawaii, fueled by a postcard from a friend vacationing there, an image in a magazine, or a movie scene? With the popularity of the Oscar-nominated film "The Descendants," crowds of travelers are being drawn to Hawaii by the film?s portrayal of what the author of the novel inspiring the film calls "the real Hawaii" ? Hawaiians struggling with workaday concerns (health problems, marital difficulties and arguments over an inheritance) amidst gorgeous island backdrops.

?It?s ironic that I wrote this book that shows other sides of Hawaii, that?s not so pretty and glamorous, and that?s why people want to come,? said author Kaui Hart Hemmings, a Hawaii native. ?I don?t think Hawaii has ever been captured on film, as it really is, until now.?

?It?s almost become like our modern-day ?South Pacific,? said Kauai Visitors Bureau executive director Sue Kanoho, comparing the film to the blockbuster 1958 musical shot on Kauai. Tour operators note the influx of visitors looking for scenes from the film, and the visitors bureau will begin surveying visitors on whether ?The Descendants? inspired their trip.

Hemmings and Kanoho both say director Alexander Payne worked to make the film look like their lives. ?He really took time to get it right,? said Kanoho, ?and gave a glimpse into modern-day Hawaii life. The film touches upon that it?s more than a sun and surf location, but really about the legacy of the land and the people.?

Protecting Hawaii?s natural beauty is a central drama in the film ? and in real life. ?People are very protective of the land, what you call the ?aina?? in Hawaiian, said Kanoho. ?The challenge is about having people come and experience the beauty, but then really understanding and respecting what [the land] means to the people here.?

The Hawaiian Islands are the perfect vacation destination for travelers of all types.

Kanoho says the conundrum over what to do with inherited land ? sell for the financial windfall, or keep for the islands? posterity ? is faced by many Hawaiian families. In fact, the property owned by the fictional King family in ?The Descendants? is a real cattle ranch: the former sugarcane plantation called Kipu Ranch, near Lihue, Kauai, is still owned by descendants of former Kauai governor William Hyde Rice, who bought the land from the Hawaiian monarchy in the 1870s.

For the view George Clooney?s character showed his daughters of their family?s land, visitors (including people with mobility issues and children at least five years old) can hop an all-terrain vehicle with Kipu Tours?for a three-hour ranch and mountain tour or a four-hour waterfall picnic tour. And Roberts Hawaii runs the Hawaii Movie Tour?on Kauai, highlighting spots shown in ?Jurassic Park,? ?Pirates of the Caribbean? and others, including Tahiti Nui, the restaurant and bar where Clooney?s and Beau Bridges? characters share a drink.

Her warm friendly people, inspiring natural beauty and unique culture draw people to the immaculate shores of Oahu.

On Oahu, where the fictional King family lives, author Hemmings recommends tourist favorites and stops off the beaten path. ?If this [film] gets people to venture out and get to know more, that?s a big score,? Hemmings said. Hemmings loves the bustle of Honolulu?s Chinatown?and the ?pockets of quiet? that can still be found in Kailu, where she lives, and on the north shore of Oahu, where visitors can watch surfers ride enormous waves.

Not to be missed, Hemmings says, is the volcano at Haleakala?? even if it is so touristy that hard-core hikers mix with Japanese tourists in high heels. ?It?s a big volcano ? there?s snow. You can hike down into the crater, but you really feel like you?re walking into the moon.??

Certain Seniors at Risk for Infection After ER Visit: Study (HealthDay)

MONDAY, Jan. 23 (HealthDay News) -- Seniors in long-term care facilities have a roughly threefold increased risk for respiratory or gastrointestinal infections if they visit a hospital emergency department in the fall, winter or spring, according to a new study.

Canadian researchers looked at 1,269 elderly residents of 22 long-term care facilities in Toronto, Ontario, and Montreal and Riviere-du-Loup, Quebec between September 2006 and May 2008.

The investigators noted that they focused on non-summer months because that's when higher rates of respiratory and gastrointestinal infections occur.

During the study period, 424 of the seniors visited an emergency department for a variety of conditions other than acute respiratory and gastrointestinal symptoms, and 845 did not go to an emergency department.

The seniors who went to an emergency department had a higher rate of chronic illnesses and tended to be less independent than those who did not go to an emergency department.

The researchers found that seniors who visited an emergency department were about three times more likely to develop an acute respiratory or gastrointestinal infection in the week following the visit, but only if there was no outbreak in the resident's facility.

The incidence of infection was 8.3 per 1,000 resident-days among those who visited an emergency department, compared with 3.4 per 1,000 resident-days among those who didn't go to an emergency department, according to the study published in the Jan. 23 issue of CMAJ (Canadian Medical Association Journal).

"Once systemic reasons for the transmission of infection in emergency departments are understood, interventions to reduce the risk should be studied," Dr. Caroline Quach of Montreal Children's Hospital, McGill University Health Center, and colleagues, wrote in a journal news release.

"In the meantime, considerations should be given to the implementation of additional precautions for residents for five to seven days after their return from the emergency department," the study authors added.

Starbucks to sell beer, wine in select stores

Starbucks Corp plans to begin selling beer, wine and more upscale food in a small number of cafes in Atlanta and Southern California by the end of this year as it explores an expansion beyond morning coffee and afternoon pick-me-ups.

Starbucks is planning to add beer, wine and food such as savory snacks and hot flatbreads to the menus in four to six outlets in both Atlanta and Southern California.

The world's biggest coffee chain started selling those items at a Seattle cafe in October 2010. Five stores in the Seattle area and one in Portland, Oregon, now offer the extended menu.

Late last year, Starbucks announced similar plans for five to seven Chicago-area cafes by the end of 2012.

MELBOURNE, Australia ? For the first time in four decades, there will be no American man in the fourth round of the Australian Open. John McEnroe thinks he knows why.

The seven-time Grand Slam champion, who is working as a TV analyst at Melbourne Park, suspects a lack of passion.

"You could certainly wonder whether our guys are as hungry as some of the others," McEnroe said on Fox after John Isner lost to 18th-seeded Feliciano Lopez in the third round Friday. "It really comes down to the individual. If people have heart or desire, that to me is more important than any shot."

This is the first time no U.S. player reached the men's fourth round at the Australian Open since 1973 ? when no Americans traveled to the tournament.

Isner, the last American man left in the singles draw, lost to Lopez 6-3, 6-7 (3), 6-4, 6-7 (0), 6-1. It was the 6-foot-9 Isner's second straight five-set match after he knocked out David Nalbandian in a 4-hour, 41-minute thriller.

"It's very ugly, to be honest, to have no one in the round of 16," Isner said. "We've got to try to rectify that next time the big tournaments roll around.

"It's very disappointing. That's not a good effort from the Americans in this tournament. I knew going in today I was the last one left and I wanted to keep on going, but just didn't happen."

Instead, Lopez earned a fourth-round match against Rafael Nadal, who won the 2009 Australian Open and has 10 major titles to his credit.

The last American man to win the Australian Open was Andre Agassi in 2003. No. U.S. man has won a major since Andy Roddick at the 2003 U.S. Open.

"We've been spoiled with the success we've had in the past," McEnroe said.

At least the Americans still have Serena Williams, the 13-time Grand Slam champion who is on a 16-match winning streak at Melbourne Park. She won titles in 2009 and 2010 but missed last year because of injury. On Saturday night, the five-time champion will play Greta Arn of Hungary in the third round.

Before that, defending champion Novak Djokovic will take on Nicolas Mahut of France at Rod Laver Arena.

Wimbledon champion Petra Kvitova was on court for 38 minutes Saturday before advancing to the fourth round. The second-seeded Czech was leading 6-0, 1-0 on Rod Laver Arena when Maria Kirilenko retired with a left thigh injury.

It was the fourth retirement of the women's tournament.

Kvitova, one of four players who could end the season's first major with the top ranking, will face Ana Ivanovic of Serbia or unseeded American Vania King in the Round of 16.

When four American men reached the fourth round of the U.S. Open in September ? only two majors after no Americans reached the round of 16 at the French Open ? Roddick talked about a "healthy jealousy" contributing to the improvement. There hadn't been four American men into the fourth round at the U.S. Open since 1995.

It didn't last long in Melbourne. Among the best contenders, No. 8 Mardy Fish lost to Alejandro Falla of Colombia in the second round before Roddick hurt his right hamstring and had to retire from his second-round match with former No. 1 Lleyton Hewitt.

Ryan Harrison took No. 4 Andy Murray to four sets before losing; Ryan Sweeting lost to No. 5 David Ferrer; and Donald Young lost to qualifier Lukas Lacko.

"People expect us to contend for Grand Slams," McEnroe said. "I think there's a lot of tennis fans, but to have an American contending and winning majors, that would make a big difference."

The pressure of being last man standing got to Isner on Friday when he lapsed in the last set after dominating the fourth-set tiebreaker.

"It just got away from me. I just wasn't as sharp as I needed to be," he said. "It just kind of spiraled out of control there, and it just ... I couldn't climb out of the hole I dug."

"I honestly felt like it was more mental than anything," he added. "Just like the whole match I wasn't really that sharp."

Now he has to consider a Davis Cup series next month against a Swiss team that could feature Federer and Stanislas Wawrinka.

"That's not an ideal draw for a first round. But, yeah, it's going to be tough," Isner said. "It's away, and, you know, more than likely I think if Roger plays we're probably the underdogs."

Djokovic started 2011 on a 41-match winning streak and finished it with the No. 1 ranking and three of the four major titles. His two main rivals are already through to the third round on the other side of the draw.

Nadal had a 6-2, 6-4, 6-2 win over Lacko, the last qualifier in the draw. Apart from having his right knee heavily taped, he showed no sign of pain from the bizarre injury ? he hurt his knee while sitting in a chair at his hotel ? that made him think he might miss this tournament.

"The knee is fine ... being in the fourth round without losing a set, it's fantastic news," he said.

Federer followed with an almost flawless performance in a 7-6 (6), 7-5, 6-3 win over 6-foot-10 Croatian Ivo Karlovic. Federer will play Australian teenager Bernard Tomic on Sunday in the fourth round. Tomic, a Wimbledon quarterfinalist last year, had a tough 4-6, 7-6 (0), 7-6 (6), 2-6, 6-3 win over 13th-seeded Alexandr Dolgopolov of Ukraine.

An early rematch of last year's women's final is already in place, with defending champion Kim Clijsters and Li Na both winning Friday night to set up a meeting in the fourth round.

Clijsters advanced with a 6-3, 6-2 win over Daniela Hantuchova. Li didn't even finish four games before Anabel Medina Garrigues quit with a badly sprained right ankle.

Medina Garrigues twisted her right ankle in the second game and needed treatment on the court. Li won the first three games and had taken the first two points in the fourth game on Medina Garrigues' serve when the Spaniard hit the ball into the air with her racket. She then went ? in tears ? to the net to retire.

"It was really tough, because she tried to continue to play, so I don't know (if) it's like real or fake ... some players they do that," Li said. Then, "I saw she couldn't run and she started to cry. I have to say I am so sorry for her."

The winner of the Clijsters-Li match will likely face a quarterfinal against top-seeded Caroline Wozniacki, who has not dropped a set in advancing to the fourth round as she continues her quest for a first Grand Slam title. She beat Monica Niculescu of Romania 6-2, 6-2, while third-seeded Victoria Azarenka defeated Mona Barthel 6-2, 6-4.

Wozniacki will next play former No. 1 Jelena Jankovic, who beat Christina McHale of the U.S. 6-2, 6-0.

That left just Williams and King as the only Americans in singles at the Australian Open. King plays Ivanovic on Saturday.

Microsoft says it will fix Xbox color and streaming resolution issues - but not when (Digital Trends)

xbox metro updateXbox recently upgraded its dashboard, adopting Windows metro style UI. It was a necessary change, as Microsoft needs to align its products with its operating system. However there were early complaints that post-upgrade, users were seeing inaccurate colors and poor HD streaming quality.?

And according to EuroGamer, Xbox was ignoring these complaints from beta testers. So why would Microsoft issue an unfinished product? Consider the fact that there is building pressure for console systems to fill many voids: not only are these devices used for gaming, but they?ve becoming the digital hub of the living room, used for streaming content and watching DVDs (and in the case of the PS3, Blu-ray movies). Xbox and PlayStation don?t only have each other and Nintendo to compete with?they have smart TVs with built-in app and Internet access as well.?

The brand has also been under pressure to release its metro UI update and offer more connectivity for Windows Phone 7 users?it needs to be able to leverage everything it can for its mobile platform, which has struggled to add more apps.?

So it seems that Microsoft prematurely pushed the update, and now there is no word on when it will be fixed?just that it will be. ?We?re aware of the colorspace issue w/some Xbox video apps & are working on a fix,? said Xbox Live programming director Larry Hyrb via Twitter. ?No ETA yet but we hope to have an update soon.?

The fact that some Xbox 360 users are dealing with poor color reproduction and the inability to play 1080p content at native resolution on the dashboard player definitely puts Xbox at a disadvantage. These malfunctions have slid (somewhat) under the radar for awhile, but it?s soured by how long the company is taking to address it all and give a real timeline on when an update will be issued.?

Europe exhales after another good week (AP)

FRANKFURT, Germany ? Europe has taken a step back from the brink.

Three weeks into the year, borrowing rates for debt-saddled countries have fallen to more manageable levels. Auctions of government debt have gone better, a sign of increased investor confidence.

And while it may have been an embarrassment, especially to France, the recent downgrade of nine European countries by credit ratings agency Standard & Poor's has been met with a shrug in financial markets.

All this is in stark contrast to the final weeks of last year, when countries such as Italy, Spain, Portugal and Greece watched helplessly as the costs of managing their debt spiraled ever higher, and governments fell in Athens and Rome.

High hurdles remain: Greece must still cut a deal with its private creditors, to say nothing of the long-term problems ? massive debt, uncompetitive economies and the prospect of years of cutbacks in public spending.

But for the moment, the continent is exhaling.

Portuguese Finance Minister Vitor Gaspar, after his country successfully sold euro2.5 billion of its national debt on Thursday, ventured that it was "a sign that we may be coming to a turning point."

Among other good news in the past week:

? Despite now having an AA+ credit rating, France easily sold euro9.5 billion, or about $12.2 billion, in bonds at interest rates lower than in previous auctions when its rating was AAA. The sale eased fears that S&P's downgrade of France would hurt the finances of the continent's No. 2 economy. France sold four-year bonds at 1.89 percent, down from 2.32 percent in November, and 10-year, inflation-linked bonds at 1.07 percent, also down from 2.32 percent.

? Spain raised euro6.6 billion ($8.5 billion), far more than its initial target of euro3.5 billion to euro4.5 billion. It agreed to pay 5.4 percent on its bonds, down from 5.54 percent in the last such auction in December. Demand was twice what was being offered.

? Stock indexes in Britain, France, Germany, Italy and Spain ? plus the Dow Jones industrial average in the United States ? have climbed back close to their levels from last August, when the crisis spread to Italy and took a turn for the worse.

The European Central Bank, chief monetary authority for the 17 countries that use the euro currency, gets some of the credit for sending cash flowing to banks ? and through them, it appears, to troubled countries.

In December, the ECB said it would lend banks unlimited amounts of money to stabilize them. It also said it would lower the interest rate on the loans to 1 percent, extend the maximum term from one year to three and accept collateral of lower quality. The banks responded by borrowing euro489 billion ($632.6 billion) in three-year loans at a low interest rate, currently 1 percent.

The banks appear to have used at least some of that money to buy the bonds that governments have been selling almost daily. The extra demand at the bond auctions also helps bring down the interest rates on the bonds.

Stefan Schneider, chief international economist at Deutsche Bank, says the ECB "is now the main source of financing" for the troubled countries' banks "and gives these banks the opportunity to invest in the government bonds of their own countries."

Another ECB all-you-can-eat credit offering is slated for Feb. 28.

The central bank has refused pleas to expand its limited program and buy government bonds itself on the open market. It says countries need to cut debt themselves and not expect a central bank bailout.

Members of the ECB governing council have cited law that prohibits the bank from financing governments. Some analysts say the massive bank loans appear to be doing exactly that, just indirectly.

Schneider, though, cautioned that there is no "magic bullet" to solve the crisis.

"I think we will not be able, even with the advantage of hindsight, to indicate the point in time when the crisis ended," he said.

On Friday, stocks in Europe mostly held their gains for the week, waiting for the outcome of Greece's negotiations with its creditors on a deal to cut the face value of up to euro200 billion in debt by 50 percent.

A deal in Athens would allow the country to receive a second bailout package from other European governments and the International Monetary Fund, and cut Greece's debt from an estimated 160 percent of its annual economic output to 120 percent by 2020.

That is still painfully high, but without the help, Greece will not be able to pay euro14.5 billion ($18.8 billion) in debt due March 20. A Greek default would send borrowing costs higher across Europe and could trigger chaos in the global financial system.

Even with a deal, Greece could default in coming years. An IMF review in December conceded that 120 percent is at the upper end of what is sustainable, and only if Greece's economy starts growing again after five years of recession.

And Greece is just one potential problem. Many countries are either headed for recession or stuck in deep ones, which will make debt reduction even tougher.

While Portugal, Greece and Ireland could be bailed out by the other euro countries and the IMF, Italy is considered much too big to rescue for any substantial length of time.

The new prime minister, Mario Monti, has promised to shake up what he calls an over-regulated, underperforming economy, accelerate economic growth and reduce the country's debt burden, which is also 120 percent of its annual economic output. But he faces tough political opposition.

Spain has taken some steps to loosen regulations on hiring and firing but has no clear growth model after the collapse of its real estate bubble. Unemployment is 22.9 percent, and for people under 25, it's a staggering 49.6 percent.

Bank stocks got a big bounce Thursday after stronger-than-expected bank earnings in the United States and word that Germany's Commerzbank could meet new requirements to boost its capital buffers without needing government help.

This week's stock rally across Europe left Germany's DAX 30 9.5 percent higher over the past month, France's CAC up 8.7 percent and the FTSE 100 in London up 5.9 percent.

Investors are all too aware that previous respites from market turmoil have turned out to be temporary.

For now, investors can increase their holdings of risky assets, says Joerg Kraemer, chief economist at Commerzbank. But he had an ominous warning this week in a note to investors.

"The ECB can only buy time," he said. "With many problems still unresolved in Spain and Italy, the sovereign debt crisis is likely to continue, and the economic recovery following the recession is likely to be lackluster."
